• Chromebook agreement

• PE Waiver (Middle school only)

• Digital Registration Forms

Students who are brand new to Putnam County School District should fill out the following packet. If you wish to fill out the forms online, that link can be found here

Students who have been in Putnam county schools but are new to Q.I. Roberts must fill out the following packet, you can skip page 1. If you wish to fill out the forms online, that link can be found here

Students who were enrolled at Q.I. Roberts last year only need to fill out the parts of the packet if there are changes to be made and all of the releases (pg 12-15 of PDF) If you wish to fill out the forms online, that link can be found here
